Parents of young adult with cerebral palsy seek Care Coordinator for two hours per week to assist in overseeing services in home and community. Responsibilities include communication with government disability systems and local service providers (i.e. medical, therapeutic, staffing agencies). Background in healthcare, social work or disability services preferred. This is not a direct support position, but individuals with experience in direct support are encouraged to apply.
