repair on old cast iron stack

Has anyone run into rusted spots or holes in cast iron stack pipes when doing renovations in old HP houses? This contractor just did when drilling out the old bathroom floor. It involves major expensive repair. Does insurance ever cover this? Would you recommend any plumber to do the work? thanks, Irina
